Physician founded and located in Denver, Colorado, Snap eHealth is a health tech company dedicated to improving patient outcomes and practice workflows to increase value-based care performance. Current healthcare processes are antiquated, redundant, and ripe for disruption. We are determined to automate and streamline these processes to help physicians escape the arduous administrative tasks they deal with daily so they can focus on what they do best—providing excellent patient care.

We have developed a suite of software solutions to help primary care physicians manage all aspects and administrative tasks surrounding Annual Wellness Visits for their Medicare patients. Our patient engagement tools capture health data from patients to help primary care physicians create a customized preventative care plan for each patient and remind patients to follow through on their care plans.

Working behind the scenes, our platform facilitates interoperability between disparate groups of healthcare providers and allows for a seamless referral process from primary care physicians to specialists.
